ACKNOWLEDGEMENT OF POLICIES AND PROCEDURES

Assumption Of Risk, Release, And Waiver Of Liability Agreement:

RELEASE AND WAIVER. In consideration of my child being accepted into the Albuquerque Youth Symphony Program (“AYSP”) and granted access to the AYSP facility, located at 4407 Menaul Blvd. NE, Albuquerque, New Mexico 87110 (“Facility”), I do hereby forever release, waive, discharge, and covenant not to sue AYSP, its past, current, and future officers, directors, employees, members, volunteers, contractors, representatives, owners, affiliates, agents, successors, and assigns (“Releasees”) from any and all damages, injuries, losses, liability, claims, causes of action, litigation, or demands, including but not limited to those for personal injury, sickness, or death, as well as property damages and expenses, of any nature whatsoever which may be incurred, directly or indirectly, now or in the future, in connection with my spending time in the Facility. I promise not to sue Releasees for any of the foregoing.
