The Musician’s Handbook is your guide to AYSP policies, procedures, and program information. All students and families are required to read through the handbook annually and refer to it throughout the year.
I have read and understand the “School Music Participation” policy on pg7 of the Musician’s Handbook. All members of AYSP must be enrolled as regular members of their curricular school band or orchestra, if available, for the duration of their participation.
I have read and understand the “Attendance Policy” on pg8 of the Musician’s Handbook.
I have read and understand the “Behavior Expectations” on pg9 of the Musician’s Handbook.
I have read and understand the “Eligibility” policy on pg16 of the Musician’s Handbook.
I have read and understand the family fundraising requirement of $100 outlined on pg16 of the Musician’s Handbook.
I have read and understand that in addition to the family fundraising requirement, all AYSP families participate in the Luminaria Fundraiser as outlined on pg16 of the Musician’s Handbook.
I have read and understand all policies, procedures and expectations as stated in the Musician’s Handbook.
Parents/guardians of any Albuquerque Youth Symphony Program (herein after AYSP) student that wishes to participate in any AYSP program or activity must sign the Student Program Release before his/her child may begin the program.
I hereby agree to have my child participate in all AYSP rehearsals, dress-rehearsals, concerts, school tours and activities scheduled by AYSP. I assume full responsibility for any and all damages, injuries (including death), or losses that my child may sustain or incur, if any, while attending, practicing, participating or witnessing in any program, rehearsal, concert or activity occurring in or about the AYSP premises or at any offsite location. I hereby assume full risk, waive all claims and release and hold the AYSP, its instructors, or partners of said program or event, individually or otherwise, harmless for any and all claims for injuries or damages.
I am fully aware and understand that the AYSP does not have on or about the AYSP premises, or employ or contract with any medical services, provisions for ordinary or emergency medical services.
In consideration of my child’s participation in and the use of the AYSP facilities, I hereby release and covenant not to sue the AYSP, its directors, officers, employees, representatives, agents, and lessees from any and all claims resulting from any physical injury that may occur to my child while participating in any program or event sponsored by the AYSP.
Select one of the following payment options. Please review the options carefully. Payments are accepted by check (payable to AYSP), or credit card. A deposit is due by July 26, 2019 to secure a place in AYSP.
Option 1: One Payment (due by July 26, 2019)
Option 2: Two Payments (50% due by July 26, 2019 & 50% due by October 15, 2019)
Option 3: Monthly Payments (deposit due July 26, 2019, four monthly payments of equal amounts due September, 1st – December, 1st, 2019. Monthly payments must be made by credit card and automatically payments will be set up on the 1st of each month)
